Job Summary:The RN Care Coordinator is responsible for providing professional nursing care, following established standards and practices as part of the patient care team. The coordinator assists physicians and staff with the daily coordination of care by organizing and expediting treatment plan of care activities, promoting communication and collaborative coordination among care providers, and providing information and education on community resources.
Essential Functions & Responsibilities:
Provides daily operational oversight at the assigned location and ensures adequate staffing.
Serves as a resource for staff and assists with the resolution of problems/concerns.
Performs general nursing care to patients. Performs phone triage duties following established protocols.
Obtains a thorough and complete medical history for all patients at their initial visit.
Assists with rooming patients and preparing exam rooms and equipment to facilitate treatment, as needed.
Obtains patient information and performs appropriate chart documentation. Administers prescribed medications and treatments as instructed by the physician.
Educates patients on related risks and testing options. Communicates test results to patients as directed by physician.
Assists with scheduling of tests and/or treatment at the office or another facility.
Identifies and screens high-risk patients for care coordination.
Coordinates care of patients with clinical partners; helps patients advance towards realistic and desirable outcomes. Organizes and expedites treatment plan of care activities. Communicates activity status updates regarding treatment plans with clinical partners.
Provides information and education on community resources to patients and their families.
Gathers insurance Prior Authorizations. Completes forms/FMLA paperwork
Performs other duties as assigned.
Qualifications:
Associate’s degree in nursing required.
Valid RN Licensure in the Commonwealth of Virginia required.
CPR certification required.
Minimum of two years of clinical RN experience required; with experience in the applicable clinical specialty preferred.
Experience with computer technology and electronic health record software required.

Physical Requirements: Constant (67-100% of workday) walking, standing, use of arms and hands; frequent (34-66% of workday) bending, stooping, kneeling, and squatting; ability to lift 35 lbs.; ability to push and pull 20 lbs.; auditory and visual skills to include color determination; ability to work in confined spaces; ability to reach above/overhead.
Mental Requirements: Possesses critical thinking and analytical skills. Ability to multi-task. Ability to communicate effectively and collaborate with a multi-disciplinary team. Capacity to cope with difficult situations. Ability to tolerate irregular hours including evenings, nights, and weekends.
Environmental Requirements and Exposure Hazards: Potential risk of exposure to radiation and toxic chemicals. Potential for exposure to bloodborne pathogens; must be able to wear appropriate personal protective equipment.
*Clinical positions in Mary Washington Home Health and/or Hospice must also meet driving requirements.
