Job Summary: The Birth Registrar collects data from birth certificate worksheets, verifies parental paternity and submits records electronically to Vital Records. The Birth Registrar ensures that all reports (typed and submitted) are accurate based upon information obtained from worksheets and verified with the patient.

Essential Functions & Responsibilities:

  • Visits each patient room, explains the birth certificate worksheet, the process of AOP (If not married), and payment methods.
  • Assists parents or unwed mothers as needed in completing paternity papers and obtains signatures from the parents and management. 
  • Collects and verifies Vital Statistics Data from the Birth worksheet after completion.
  • Types birth certificates using VVEST/ VDH software for electronic birth certificates.
  • Notarizes paternity paperwork and other legal documents as needed.
  • Sends signed application for Birth Certificate, paternity paperwork, and request for social security card to the Department of Vital Records for processing. Explain and provide parents with Proof of Birth Letter.
  • Sends copies of birth certificate paperwork to HIM (or designee) for management signature
  • Maintains an official birth certificate completion log/paper backup of information in appropriate system.
  • Reports all paternity documents Bi-weekly to the Virginia Paternity Establishment Program.
  • Prints out hard copy for review and signature by the mother, placing a copy on the mother’s chart.
  • Identifies when fetal death forms are needed. Sends monthly report of all fetal deaths routinely every month. Faxes information to VDH and management.
  • Completes demographic information on forms and forwards to Health Information Management.
  • Sends finalized forms to the Virginia Department of Health, Division of Health Statistics.
  • Collect payment for birth certificate processing.
  • The Birth Registrar has the right to refuse if the appropriate information is not provided for verification.  
  • Works closely with hospital case manager and bereavement coordinator.
  • Monitors stock of supplies and orders from VVEST as needed.
  • Performs other duties as assigned.

Qualifications:

  • High school diploma or equivalent required.
  • Required to obtain Notary Public access within 60 days of hire or placement.
  • Six months of relevant experience is preferred.
  • Demonstrates effective communication, customer service, confidentiality, and team building skills.
  • Knowledge of Virginia State Laws regarding married/un-married parents.
  • Knowledge/ understanding of legal name changes.
  • Explain the Statute of Limitations to Mothers who are not married and choose not to add the intended Father of Baby to the Birth Certificate.
  • Knowledge of state laws for surrogacy process, adoptions, fetal demise, fetal death.
  • Knowledge of DNA testing related to intended father rights and locations.
