Job Summary:This position provides a centralized intake and customer service function to optimize referrals and satisfaction with agency responsiveness. Duties and responsibilities are performed according to standards outlined in the competency based evaluation. The incumbent in this position will be responsible for compliance with all federal and state laws and regulations governing the privacy and of patients' protected health information.

Essential Functions & Responsibilities:

  • Coordinates and facilitates intake functions and patient registration processes in the electronic record system while providing excellent customer services to internal and external stakeholders to include community physicians, facilities, insurance companies and other referral sources.
  • Manages and tracks all incoming patient referrals via phone and fax while ensuring all necessary documents are received to start patients’ home care experience. Provides verification of insurance, obtains authorization of insurance, and completes follow-up to convert inquiries into service requests for agency as needed.
  • Performs routine data analysis to monitor referral patterns/trends and submits findings to clinical leadership. Assists department leadership in proactively identifying and addressing administrative and operational improvement opportunities that promotes high quality and effective processes and procedures.
  • Coordinates seamless initiation of patient care services by collaborating with the clinical, billing, and administrative teams. Performs follow-up activities in support of patient care and assists Homecare Coordinator(s) and Clinical Supervisor with scheduling of new admissions as needed.
  • Make independent decisions based on business processes to qualify MWHC services for appropriate referrals based on business specifications and regulations.
  • Performs other duties as assigned.

Qualifications:

  • Valid LPN License from Virginia
  • One year experience in homecare environment, physician office, and/or medical case management strongly preferred.
